18-23月龄儿童甲型肝炎减毒活疫苗和麻腮风联合减毒活疫苗单独和同时接种的免疫效果研究Immunogenicity of hepatitis A attenuated live vaccine, and measles, rubella and mumps combined attenuated live vaccine for standalone and simultaneous vaccination in 18-23-month-old children

摘要(Abstract):
目的评价18-23月龄儿童甲型肝炎(甲肝)减毒活疫苗(HepA-L)、麻腮风联合减毒活疫苗(MMR)单独和同时接种的免疫效果。方法采用多中心、随机、对照设计,招募无甲肝疫苗免疫史且完成首剂次含麻疹成分疫苗的18-23月龄儿童,随机分为3组,分别接种HepA-L、HepA-L与MMR、MMR,检测每名儿童免疫前、免疫后1个月血清甲肝、麻疹、腮腺炎、风疹IgG抗体,分析免疫后抗体阳性率和阳转率。结果 HepA-L组儿童免疫后甲肝抗体阳性率、阳转率分别为81.3%(148/182)、80.3%(139/173)。MMR+HepA-L组儿童免疫后麻疹、风疹、腮腺炎、甲肝抗体阳性率分别为98.3%(178/181)、92.3%(167/181)、78.5%(142/181)、77.9%(147/181),阳转率分别为90.0%(9/10)、86.7%(26/30)、78.2%(129/165)、75.5%(123/163)。MMR组儿童免疫后麻疹、风疹、腮腺炎抗体阳性率分别为94.0%(157/167)、84.4%(141/167)、77.2%(129/167),阳转率分别为87.5%(14/16)、83.7%(36/43)、78.3%(123/157)。结论 18-23月龄儿童单独和同时接种HepA-L和MMR均具有良好的免疫效果;同时接种对各疫苗成分的免疫效果没有影响。
